Fe elings of shame or humiliation around dependency c an provoke defensive withdrawal\, making even well -intentioned care feel intrusive or persecutory. A s a result\, patients often reject services precis ely when engagement is most crucial. Risk can esca late rapidly if these reactions are misunderstood or avoided&mdash\;whether out of fear of confronta tion or an overcorrection toward preserving autono my. In such moments\, thoughtful engagement in the face of resistance is not simply persistence\; it is a clinical imperative.\n \n \n Violence risk a ssessment is never an exact science. Risk is dynam ic and can shift suddenly\, especially in communit y contexts where contact with patients is limited. Staff are often under immense pressure to make fi nely balanced decisions\, typically with incomplet e information and under emotionally charged condit ions. What kinds of structures best support clinic ians in navigating these conflicts&mdash\;between the risk of overreach and the danger of inaction? The anxiety surrounding such decisions can impair clinical judgment\, leading to either excessive ca ution or unwarranted deference to autonomy. Yet re sponsibility for these decisions cannot be deferre d. Paradoxically\, avoiding action in the name of independence may undermine the very care and safet y that ethical practice is meant to protect.\n \n \n \n \n Marcus Evans \;is a \;Fellow of t he British Psychoanalytical Society-with a long-st anding interest in applying psychoanalytic thinkin g in mental health settings. Before retiring from the \;NHS\, \;he \;was \;a psychia tric nurse and \;an \;adult psychotherapis t. \;He was a founding member of Fitzjohn&rsqu o\;s service for the treatment of patients with&nb sp\;severe and enduring mental health issues and&n bsp\;personality disorders and clinical lead \ ;of \;the adult and adolescent services \; at the Tavistock \;for five years.  \;He i s the author of \;three \;books. \;&ls quo\;Making Room for Madness in Mental Health&rsqu o\;\, \;&lsquo\;Psychoanalytic Thinking in Men tal Health Settings&rsquo\; \;&\; \;&ls quo\;Gender Dysphoria: A therapeutic model for wor king with children adolescents and young adults&rs quo\; \;a book he wrote with his wife Susan Ev ans.\n \n Dr Olive Burke is an Adult Psychoanalyst (BPAS) working in full-time private practice. She has over 30 years experience of working in Mental Health\, including as a Consultant Child &\; A dolescent Psychiatrist at the Tavistock &\; Por tman NHS Trust for nearly 20 years prior to her re tirement from NHS. She has always been interested in using psychoanalytic insights to think about ps ychiatric issues at a deeper level &\; to infor m treatment approaches. She continues to be invest ed in the development &\; support of young prof essionals\, particularly within the NHS.  \;\n \n Dr Lisa Conlan \;is a Consultant Psychiatr ist and Psychoanalyst working in community mental health with the South London and Maudsley NHS Foun dation Trust. Recently qualified as a psychoanalys t\, she brings a rich\, integrative approach to he r clinical practice\, combining evidence-based psy chiatry with psychoanalytic insight. Dr Conlan als o maintains a strong academic interest in the Phil osophy of Psychiatry and Medical Education\, where she explores the interface between philosophical thought and psychiatric practice. \;\n \n Dr R onald Doctor \;is a Consultant Psychiatrist in Medical and Forensic Psychotherapy at the \;W est London NHS Trust\, and a practicing Psychoanal yst. A Fellow of both the British Psychoanalytical Society and the Royal College of Psychiatrists\, he is also a Senior Member of the British Psychoth erapy Foundation. Dr Doctor is the Chair of the In ternational Psychoanalytical Association'\;s Co mmittee on Psychoanalysis and Law and serves on th e Executive Board of the International Association for Forensic Psychotherapy.
